Clean as best you can. Then work some glue in (just plain old
white glue is fine, push it in with splinters if you have to).
After it's dry (and still warped and delaminated), which
will take a couple of days, use a hot clothes iron (with
aluminum foil over the wood, to keep the finish from
sticking to the iron) to flatten it. If you can arrange
a pressure plate over the unflat bits, apply that while
it cools down.
