Medium Density Overlay -
Exterior type plywood finished with an opaque resin treated fiber overlay
to provide a smooth surface ideal as a paint base. Recommended for siding
and other outdoor applications, and for built-ins, signs and displays,
furniture, etc. Available without grooving, with V-grooves, or in T1-11 or
reverse board-and-batten grooving.

MDO is often used for concrete forms. Try a building contractor supply
outlet.

Dave,
MDO (Medium Density Overlay) is a compressed paper "veneer" that is applied
to the surfaces of a plywood sheet, usually for the purpose of providing a
higher quality (smoother) surface for painting. It is usually a bit more
expensive than "B" or "C" surface grade plywood; but may be about the same
price as "A" surface material. The paper coating isn't usually very hard;
so it scratches pretty easily if left unprotected. Painting is the usual
protection for MDO surfaces. A good quality polyurethane varnish coating
will "soak in" and substantially harden the surface after it cures. I have
used MDO plywood for low cost door panels and, with a poly finish, as the
top for small workbenches. MDO can be pretty hard to find in most lumber
yards; but they can get it for you if you ask.
